At 5 weeks into pregnancy things are so small there is very little to see on ultrasound. Even at 6 weeks it can be difficult to see an embryo with some people. We say 5 weeks plus because without being able to measure the embryo, we can only measure the mean sac diameter which is a combination of 3 measurements of the sac to gather an approximate date, but it isn’t as accurate and measuring the embryo. At this point we can see the dark area with a small circle inside. The dark area is the gestation sac and the small white circle is called a yolk sac. The job of the yolk sac is to provide the growing embryo with nutrients until the placenta takes over later on into pregnancy. With only a yolk sac to see, we can confirm that the pregnancy is in the right place or not ectopic. The image above is now showing a 6 week pregnancy at this point is is possible to see the embryo and measure the gestational age quite well, but it is not always possible to see a heartbeat at this time.

Whilst you may get a positive pregnancy test before your period is even due it is a couple of weeks before the pregnancy is big enough to be seen on scan. In the early weeks how much we can see in very early pregnancy is different for every woman and every pregnancy. At 4 weeks gestation 2 weeks after conception when you have just missed your period we cannot detect the pregnancy on scan, all we can see is that the lining of the womb has become thicker.
Somewhere between 4 and 5 weeks, we can sometimes see a small black fluid area in the uterus but we cannot confirm it is a pregnancy sac until we can see something growing inside it. During the 5th week we can usually see a small pregnancy sac around 6mm and within it we can usually see a yolk sac. This confirms to us that this is definitely a pregnancy sac.
Even at 6 weeks we sometimes struggle to see the baby and its heart beat even when things are going fine.

An ultrasound is a highly accurate means of dating a pregnancy. In a normal pregnancy, ultrasound can provide an estimate of the gestational age to within five to seven days of accuracy. In the first trimester, doctors usually use a transvaginal rather than abdominal ultrasound. The transvaginal ultrasound provides the most accurate information, given that the early developing gestational sac and fetal pole are extremely tiny at this point. A transvaginal ultrasound can get closer to the developing pregnancy.
In a transvaginal ultrasound, the doctor or technician inserts a thin probe into the vagina.
A dating scan is one that is performed in order to establish the gestational age of 5 ½ weeks gestation means 5 ½ weeks from the first day of the last menstrual By 6 to 7 weeks gestation the fetus is clearly seen on trans-vaginal ultrasound.
A dating scan is an ultrasound examination which is performed in order to establish the gestational age of the pregnancy. Most dating scans are done with a trans-abdominal transducer and a fullish bladder. If the pregnancy is very early the gestation sac and fetus will not be big enough to see, so the transvaginal approach will give better pictures.
Dating scans are usually recommended if there is doubt about the validity of the last menstrual period. By 6 to 7 weeks gestation the fetus is clearly seen on trans-vaginal ultrasound and the heart beat can be seen at this early stage 90 to beats per minute under 6 to 7 weeks, then to beats per minute as the baby matures. Ultrasounds performed during the first 12 weeks of pregnancy are generally within 3 – 5 days of accuracy.
The most accurate time is between 8 and 11 weeks gestation. This is because the fetus is growing so quickly that there is a big difference in size from week to week. However, the accuracy of the ultrasound examination is always dependent on the skill of the sonographer and the quality of the equipment. The EDD from the early dating scan is used – if the last menstrual period is not known or is unreliable, or the dating scan differs from the last menstrual period dating by more than 5 days.
Ultrasound examinations from 12 to 22 weeks are regarded as being within 10 days of accuracy or up to 10 days earlier or 10 days later than the woman’s calculated due date. Ultrasounds performed after 22 weeks gestation cannot be used to estimate the due date of the baby because the size no longer reflects the age very well. Even average babies can differ by up to 2 to 3 “weeks of growth”;.
